Difficulty Modes in Diablo 2 affect a playthrough's challenge, this page covers a list of all difficulty settings that players can choose and information on the changes applied to the selected difficulty.
How to unlock?
In order to unlock the following difficulty, you have to complete the previous one. Once you complete Normal difficulty, you unlock Nightmare, and after you are done with it, Hell is unlocked.
Hardcore Mode
Hardcore mode can be unlocked after finishing the game on Normal difficulty. After doing this, you can create a Hardcore Character, that in essence is the same as a normal character, with the only difference that if your character dies, the game ends and you can't reload your last save.
